Features/Improvements:

Fixes:
- Removed a multi-encoded html form in discussions which may have prevented
non-ascii characters from working
- Fixed a temporary deadlock if the template page was accessed many times
- After a user requests a temporary password and logs in with it, the
previous password will no longer work
- When preserving formatting in wiki, the code/pre block was being parsed
- Ticket contact name needed to be converted to HTML for proper formatting
- OmniOutliner3 import failed if LOE value existed without "h" or if columns
had any styles applied
- Deleting a version updates the file date correctly
- Cleaned up some extra space in the wiki output
